CORONADO POP WARNER FOOTBALL AND CHEER 2012 REGISTRATION

Time: 8:30am to 1pm
FOR A COMPLETE REGISTRATION YOU MUST BRING THE FOLLOWING ITEMS WITH YOU:
- Participant and parent/guardian MUST BE PRESENT for signatures on paperwork as well as a mandatory weight for football players and uniform fittings for cheer.
- ORIGINAL and TWO copies of birth certificate
- ORIGINAL proof of residency (1)
- A recent 2 x 2 color picture for the league to keep. This must be a headshot with no hats or glasses.
- Registration fees of $200 for first player in family and $150 for each additional participant. $85 fee for flag players. $40 late fee for registrations after June 16. Scholarships are available.
- In addition, each player MUST supply a completed physical on the official Pop Warner physical form and a final (full year) report card for the 2011-2012 school year by July 23 and be available for mandatory conditioning on July 30.
